Vitamin B6 is a cofactor in more than 100 enzyme reactions, primarily involved in protein metabolism, neurotransmitter synthesis (serotonin, dopamine, GABA), and immune function. It also helps regulate homocysteine levels, and elevated homocysteine is an independent risk factor for cardiovascular disease and cognitive decline. Low B6 status is associated with increased inflammation and higher CRP levels.
How Vitamin B6 affects aging
While folate and B12 recycle homocysteine back into methionine, B6 handles the exit route: as pyridoxal 5'-phosphate (PLP), it powers cystathionine beta-synthase, which drains homocysteine into the transsulfuration pathway. The end product is cysteine — the rate-limiting ingredient for glutathione, the cell's master antioxidant. So low B6 hits aging biology twice: homocysteine rises, and glutathione synthesis, already declining with age, loses its supply line. It is why homocysteine-lowering protocols pair all three B vitamins rather than relying on folate alone.
B6 has an unusual, two-way relationship with inflammation. Plasma PLP falls during inflammatory states — the vitamin appears to be consumed at sites of inflammation — so low B6 is partly a marker of inflammaging. But the arrow points the other way too: NHANES data show higher B6 intake tracks with lower C-reactive protein, and people with elevated CRP need substantially more B6 to reach adequate blood levels. For older adults, whose baseline inflammation runs higher, the standard RDA may quietly undershoot.
PLP is the cofactor for synthesizing serotonin, dopamine, and GABA, which is why deficiency shows up as irritability, low mood, and cognitive fog rather than anything dramatic. Less known: B6 is an efficient scavenger of reactive carbonyl species, the molecules that drive formation of advanced glycation end-products (AGEs) — the cross-links that progressively stiffen skin, blood vessels, and other long-lived proteins with age. Between neurotransmitters, homocysteine, glutathione, and glycation, B6 touches more aging pathways per milligram than almost any nutrient.

Food gets most people there: chickpeas, tuna, salmon, poultry, potatoes, and bananas are all solid sources, and B6 survives cooking better than folate does. Supplements come as pyridoxine hydrochloride or pyridoxal 5'-phosphate (P5P); both work, since the liver interconverts the forms. Modest doses are the point — 2–10mg daily covers even the elevated needs that come with inflammation, ideally taken alongside folate and B12, which share the homocysteine pathway.
B6 is the one B vitamin with a real toxicity story: chronic high doses of pyridoxine cause sensory neuropathy — numbness and tingling that, ironically, mimics the deficiency it was meant to fix. The US tolerable upper limit is 100mg per day, and cases have been reported below that with long-term use. There is no benefit case for megadosing; skip the high-dose capsules common in supplement shops and stay in the low-milligram range.

What the research shows
Analyzing 2,686 US adults from NHANES, higher B6 intake was inversely related to elevated C-reactive protein, and inflammation dramatically changed how much B6 people needed: among those consuming 2–3mg daily, fewer than 10% had inadequate blood PLP when CRP was low, but nearly half did when CRP exceeded 10 mg/L. The implication is uncomfortable for one-size-fits-all RDAs — requirements are not fixed, and the inflamed (often older) body needs more.
Pooling prospective studies, blood levels told a stronger story than dietary intake: people in the highest category of blood PLP had roughly half the colorectal cancer risk of the lowest (RR 0.52), with risk falling 49% for every 100 pmol/mL increase. Observational data with a real caveat — PLP also drops with inflammation, which itself relates to cancer risk — but the gradient is striking.

Can vitamin B6 be toxic?

Should I take P5P instead of regular pyridoxine?

Why is my B6 low on a blood test even though I eat well?
