Vitamin B12 is essential for nerve myelination, red blood cell formation, and DNA synthesis. Deficiency causes irreversible neurological damage if untreated. How Vitamin B12 affects aging
B12 and folate are metabolic partners — B12 is the cofactor for methionine synthase, the enzyme that transfers folate's methyl group onto homocysteine to regenerate methionine. Without B12, folate gets stuck in its methylated form (the "methyl-folate trap"): homocysteine climbs, DNA synthesis stalls, and the methylation reactions that maintain gene expression, neurotransmitters, and myelin all run short of raw material. This is why B12 deficiency can mimic folate deficiency in blood work while being far more dangerous for the nervous system.
The nervous system is where B12 deficiency does irreversible harm. B12 is also the cofactor for methylmalonyl-CoA mutase, and when it runs low, methylmalonic acid accumulates while myelin — the insulation around nerve fibers — degrades. Symptoms progress from tingling feet to balance problems to cognitive impairment, and if caught late, some damage stays. Aging quietly sabotages B12 absorption. Extracting B12 from food requires stomach acid and intrinsic factor, and atrophic gastritis — common after 60 — plus acid-suppressing drugs and metformin all cut absorption of food-bound B12. Because the liver stores several years' worth, deficiency creeps in slowly and is easy to miss: serum B12 can look borderline-normal while methylmalonic acid, a more sensitive marker, is already elevated. This is why B12 is one of the few nutrients where routine supplementation after 60 has broad expert support.

For younger omnivores, food handles it: clams, fish, meat, and eggs deliver B12 in absorbable form. Vegans must supplement — no unfortified plant food provides reliable B12. Cyanocobalamin and methylcobalamin both work; cyanocobalamin is better studied and more stable. Because a small fraction of any oral dose absorbs passively without intrinsic factor, high oral doses of 500–1,000mcg daily correct deficiency about as well as injections for most people.
B12 has no established tolerable upper limit — excess is excreted, and 1,000mcg doses are routinely used safely. Adults over 60 should favor fortified foods or supplements regardless of diet, since crystalline B12 does not need stomach acid to absorb. Long-term metformin or PPI users should test periodically. Timing barely matters; take it whenever you will remember, with or without food.

What the research shows
In 168 older adults with mild cognitive impairment, two years of high-dose B vitamins (folic acid, B6, and 500mcg of B12) slowed the annual rate of brain atrophy from 1.08% to 0.76% versus placebo. In participants with homocysteine above 13 µmol/L, the atrophy rate was 53% lower with treatment. A small trial, but a rare randomized signal that a nutrient intervention can slow a structural marker of brain aging.
A follow-up analysis of the same trial found the B-vitamin effect concentrated exactly where it matters: gray matter regions vulnerable to Alzheimer's disease lost 3.7% of volume over two years on placebo versus 0.5% with treatment — as much as a sevenfold difference. The analysis supported a causal chain in which B vitamins lower homocysteine, which in turn slows gray matter loss. Benefits were clearest in people who started with elevated homocysteine.
Reviewing large population surveys, roughly 6% of US and UK adults over 60 are frankly B12 deficient and closer to 20% have marginal status, with prevalence rising with age — driven mostly by malabsorption of food-bound B12 due to gastric atrophy rather than low intake. Methylcobalamin or cyanocobalamin — which form is better?
Both correct deficiency. Cyanocobalamin is more stable, cheaper, and better studied; methylcobalamin is marketed as more natural, but the body interconverts the forms anyway. Consistency matters far more than what is on the label.
I'm over 60 and eat meat — do I still need to supplement B12?
Quite possibly. After 60, declining stomach acid impairs the release of B12 from food, so you can eat plenty and still absorb little. Fortified foods or a modest supplement bypass the problem entirely, and a methylmalonic acid test settles whether you are covered.
Can vegans get B12 from spirulina or nutritional yeast?
Not reliably. Spirulina and most algae contain pseudo-B12 analogues that are inactive in humans and can even distort blood test results. Nutritional yeast only counts when it is fortified — check the label. A standard B12 supplement is the dependable route on a plant-based diet.
Is a serum B12 test enough to rule out deficiency?
Not always. Serum B12 is a blunt instrument — values in the low-normal range can hide functional deficiency at the tissue level. Methylmalonic acid (MMA) and holotranscobalamin are more sensitive markers; ask for MMA if you have neurological symptoms alongside a "normal" B12 result.
Can you take too much B12?
There is no established upper limit — surplus B12 is excreted, and high doses are used therapeutically without toxicity. The bigger risk runs the other way: supplementing blindly can normalize blood counts while the real cause of symptoms goes uninvestigated, so test before you treat if symptoms are present.
